What Is a Stiff Shoulder?
A stiff shoulder, also known as Frozen Shoulder or Adhesive Capsulitis, is a condition in which the shoulder joint becomes painful, stiff, and difficult to move. This happens when the capsule surrounding the shoulder joint thickens and tightens, restricting normal movement.
The condition usually develops slowly and may take several months to resolve.
Causes & Risk Factors
Stiff shoulder may develop due to:
Prolonged immobilization after injury or surgery
Diabetes (one of the most common risk factors)
Thyroid disorders
Shoulder trauma or previous surgery
Stroke or neurological conditions
Age-related changes (commonly between 40–60 years)
Symptoms of Stiff Shoulder
Common symptoms include:
- Gradual onset of shoulder pain
Increasing stiffness and reduced movement
Difficulty lifting the arm overhead or behind the back
Night pain or pain at rest
Difficulty with daily activities such as dressing or combing hair
Stages of Stiff Shoulder
Stiff shoulder usually progresses through three stages:
Freezing Stage
Increasing pain with gradual loss of motion
Duration: 6 weeks to 9 months
Frozen Stage
Pain may decrease, but stiffness remains severe
Duration: 4 to 6 months
Thawing Stage
Gradual improvement in shoulder movement
Duration: 6 months to 2 years
Diagnosis
Diagnosis is based on:
Detailed physical and clinical examination
Assessment of active and passive shoulder movements
X-rays to rule out arthritis or fractures
MRI (if required) to confirm diagnosis and assess soft tissues
Treatment Options
Treatment focuses on pain relief and restoration of movement.
1. Non-Surgical Treatment
Most patients recover without surgery:
Pain-relieving and anti-inflammatory medicines
Physiotherapy and stretching exercises
Heat or ice therapy
Corticosteroid injections in selected cases
Home exercise program
Early and regular physiotherapy is key to recovery.
2. Surgical Treatment
Surgery is rarely required but may be advised if symptoms persist despite conservative treatment:
Manipulation under anesthesia
Arthroscopic capsular release
These procedures help improve shoulder movement and reduce stiffness.
Recovery & Prognosis
Recovery may take several months
Consistent physiotherapy is essential
Most patients regain near-normal shoulder function
Early treatment leads to better outcomes
