Shoulder SLAP

Shoulder SLAP Treatment

By Dr. Kanta Prasad (MBBS, MS – Orthopaedics)

What Is a SLAP Tear?

A SLAP tear (Superior Labrum Anterior to Posterior) is an injury to the labrum, the ring of cartilage that surrounds the shoulder socket. In this condition, the top (superior) part of the labrum is torn from front to back, often involving the attachment of the biceps tendon.

SLAP injuries can cause shoulder pain, instability, and reduced performance, especially in athletes and individuals involved in overhead activities.

Causes & Risk Factors

SLAP tears may result from acute trauma or repetitive stress, including:

  • Fall on an outstretched arm

  • Sudden pulling or lifting of heavy objects

  • Repetitive overhead activities (throwing, swimming, weightlifting)

  • Shoulder dislocation or trauma

  • Age-related wear and tear (degenerative tears)

  • Sports such as cricket, tennis, baseball, and volleyball

Symptoms of SLAP Tear

Common symptoms include:

  • Deep shoulder pain, especially during overhead movements

  • Clicking, popping, or catching sensation in the shoulder

  • Shoulder weakness or instability

  • Reduced range of motion

  • Pain during throwing or lifting activities

  • Difficulty sleeping on the affected side

Diagnosis

Dr. Kanta Prasad evaluates SLAP injuries through:

  • Detailed history and physical examination

  • Special shoulder tests

  • X-rays to rule out bone-related problems

  • MRI or MRI Arthrogram for accurate assessment of labral tears

Treatment of SLAP Tear
Treatment depends on the patient’s age, activity level, and severity of the tear.

1. Non-Surgical Treatment

  • Rest and activity modification

  • Anti-inflammatory medications

  • Physiotherapy focusing on shoulder stability and strength

  • Steroid injections (in selected cases)

Many patients improve with conservative management.

2. Surgical Treatment

Surgery is recommended if symptoms persist despite non-surgical treatment.

Arthroscopic SLAP Repair

  • Minimally invasive keyhole surgery

  • Torn labrum is repaired using anchors and sutures

  • Performed under anesthesia with faster recovery

Biceps Tenodesis (in selected patients)

  • Preferred in older or degenerative tears

  • Relieves pain while preserving shoulder function

Recovery & Rehabilitation
  • Sling support initially after surgery

  • Structured physiotherapy program

  • Gradual return of shoulder strength and motion

  • Return to sports typically in 4–6 months

  • Excellent outcomes when treated early

When to See a Shoulder Specialist
  • Persistent shoulder pain

  • Clicking or locking sensation

  • Pain affecting sports or daily activities

  • Shoulder instability or weakness
